vendor/patchwork you have 2 infected files:

index.php
lndex.php

How old a board are you running? patchwork was removed in phpBB 3.3.5

The ? in the URL indicates the remainder of the URL is a parameter being passed into the index.php. The html file with special characters does not exist, it is just a parameter value.
